News to Use #97 Form for Treating a minor and Changing DX codes pop ups

On Version 08.21 Russ snuck in the Consent to Treatment of a Minor form (seen below) into the merge-able standard documents section. As you may know with the merge-able documents, you have to select the document you want and the patient name, and then it merges all the pertinent information. What is unique about this document, is that it allows the parent to sign directly on the screen (if you are using a Tablet PC) then it can either be printed and saved in the patients folder, or for the greener chiropractors, it is saved, and can be accessed by going to Past Reports and clicking on the patients name and then selecting Insurance forms and it will appear in the list box to the right.

This is what the form looks like:

required but is given to provide authority to the above described agent(s) to give specific consent to any

and all such diagnosis and treatment which chiropractor, meeting the requirements of this

authorization, may, in the interest of his/her best judgment, deem advisable.

This authorization will remain effective until revoked in writing delivered to the agent(s) noted above.

I invented something that Russ built into the last update. If a patient has more that one problem, i.e. neck pain and low back pain you would probably have diagnosis codes that reflect both areas, something like: ICD-9 Codes: 723.1 Cervicalgia (neck pain), 353.2 Cervical root lesion, 353.1 Lumbosacral plexus lesions, 728.85 Spasm of muscle. When the patient presents stating that one of the problems is resolved, and you hit the remove button, this will now trigger the new DX box to flash to remind you to remove the diagnosis codes that no longer apply, i.e. the back pain is gone, we lose the 353.1 by going to Modify DX and delete the ones that have been resolved and add the ones that describe the condition that is still present.

Another helpful feature we created is that, if the diagnosis date of onset is over 90 days, the date will flash on the travel card to alert you. We assume that since all of us have to manage many things at the same time, anything that will make the documentation easier and get you paid faster is a good thing.

simple problems i have noticed

i have notice that there is a new screen that pops up when asked if the patient has been to a chiropractor before. then it asks the results. i noticed that the sentence is included in the notes but not the results (great,good etc0, there is just a small blank space there. is this just my problem or have others noticed it as well?

also with yeoman's test the text in the notes is quite weird. for example the patients name is norma. From the notes.....

I stablized the patient's Norma sacroiliaic joint with one hand while I flexed her Norma knee with the other. I then hyperextended her Norma thigh..

its goes on. i was wondering if i can fix this or is this something everyone has noticed.

New to Use #96 More that one "Case" per patient

I have frequently made the analogy that E-Z Notes documentation is like using a digital camera. You can take a great picture just by pushing a button when you take the camera out of the box, but if you read the instruction manual, you can learn some tricks that will make you a better photographer.

So it is with computer documentation. Our system, now in the hands of over a thousand chiropractors, works fine to generate a complete S.O.A. P. note, but it does much more if you dig a little deeper. In News to Use #77 http://www.websitetoolbox.com/tool/post/eznotes/vpost?id=2445663 I address a few ways that you can differentiate that same patient but a different case, i.e. Sarah Palin the cash patient and Sarah Palin the Auto accident (PI).

Russ invented a still better way to do this which you may not know about. At present, if you put in a patient, say by using Regular Visit, the system will ask for the patients first name, their last name and their gender. If you try to enter them again, it will recognize that they are already in the system and it will ask you to differentiate them from the first time you put them in by naming a case. The first time the patient was entered becomes thedefault patient and the second time could be anything you choose: Auto, MVA, PI, WC, etc.

When you create your note, you decide which of the cases you are working on. I know this sounds a bit like multiple personalities (which it may be), but it will prevent you from pulling information that does not pertain to the auto case, especially when you use the Narrative button. Another way to use this feature is for the office that is multi-disciplinary. Another healthcare provider in the same office such as a massage therapist, physical therapist or MD, can generate a note by going to Regular Visit, entering the patients first name, last name and gender. The system will recognize that patient already exists and ask if you want to choose a case now you can put, PT for example and create a note. From here on you will always be prompted to pick the patient and the case. It is little like your mother giving a description of you and then your father giving his impression of you. They are both stories about you, but from a different vantage point.

Pain Figure marking on the Daily notes

Suppose there was a way to have an AP and Lateral figure where the patient would mark where they hurt and the number of their pain that corresponds to that area and then when you printed out their daily notes a figure of the AP and Lateral Diagram with their markings on where they hurt in the patients own words would print along with the daily soap note. Just an Idea!

Dear Fellow Chiropractors:

Many of you called me to say that you could not access your E-Z Notes software due to the new HIPAA password protection that was released on 09-12-08 with version 08.19. As you learned, the password to get in, is the same one you use to get into the Set Up screen (the factory default is eznotes) We are sorry if this presented a problem, but if you had read Russ list, it was mentioned in item #9, perhaps Russ was right that not everyone is reading our e-mails. (No disrespect meant, just a fact of fast paced life)

Another new feature (#5 of last weeks list) is the insurance verification screen. As I understand it, this is how it was designed to be used. Lets say you have a new patient scheduled, and your front desk does an insurance verification call. They can find out which therapies are covered by the policy, which may factor into what you recommend for the patients care, and use this new screen to list the information. For example, you notice that the patient has excessive lateral heel wear on his shoes and you observe no metatarsal arch (flat feet) and think this guy is a Pronator and he will hold his adjustment better if I make him a pair of orthotics. You can now see at a glance, if his insurance will cover orthotics (under notes section for example). If they are not covered, the patient may opt to pay out of pocket, but you can instantly let him know. The insurance verification screen is accessed from: Main Screen > Edit Patient Info > 9^th button down on the right says Ins. Verification. The information is filled out by clicking the yes and no circles and popping in a few other numbers. Now, at present, for the doctor to access this info, the doctor can either go into Edit patient info and select Ins. Verification or he can get this from the super bill, where you will see a green box on the upper right portion of the screen called Check Insurance Verification. I have asked Russ to also add it to E-Z options, which might be a bit easier to answer a question while doing the note. In any case if you begin having your staff add the data to E-Z Notes, it may prevent unpleasant surprises and angry patients.

I was told by one of our doctors that the code 76140 might also serve if offering an opinion about X-Rays taken outside your office, so I called our resident expert David Klein 215-957-1035 (who can be hired to help with compliance and protection from audit) He cautioned that the use of say 72040-26 may be a red flag and prompt an audit. Each doctor will have to decide for him or herself.

Is it possible when you choose "other" for time modality was performed and input lets say thirty minutes of manual therapies that it can automatically put two units in the superbill. It seems like I always need to change the 1 unit that is there to the proper amount of units I have performed on the patient.

HP Touchsmart computer

Just bought a new HP 22" Touchsmart all in one computer. This thing is incredible. Looks just like the Imac all in one. Sleek and black with wireless keyboard and mouse. The touch screen works really well with ez notes. 2 models to choose from. 1200 at office depot and 1400 for the bigger one, 320 vs 500 gigs. Got mine at Sams for 1299 same as the 1400 dollar one without the cable tv hookup.

Dear Fellow Chiropractors:

I should have known this but because I didnt, there is a chance that some of you might find this information helpful. Sometimes when I am racing to finish a Regular Visit note, I find myself looking at the type written end product and I remember that I performed some modality and I forgot to mention it in my note. As long as you have not clicked the red button that is marked Exit/save, you can go back and change the note, heres how. At the bottom left corner of the type written page is a button marked, Back to SuperBill. As you would imagine, this will take you back to the Superbill, and from there you will see a button in the bottom right corner to the left of the green button that is marked Done, it says Back on it. I dont know why I never saw this before, but it has become my savior. This Back button takes you back to the Travel Card. From there, I can edit any of the screens and add things that I forgot. I simply hit Done two times and I am back at the finished note. Its brilliant!

I think that there are two places in the SOAP note process where you should pause and think. One is at the Superbill, because you want to make sure that you have clicked all the procedural codes correctly, and the other is while looking at the final note, right before you hit Exit/Save, so that you can go back if necessary. If youre happy with the note, hit the red button and move on.

When Russ and I were at the Florida Chiropractic Association we met a lot of talented chiropractors, I want to introduce you to two of their companies here. Dr. Todd Mexico and his partner Dr. Brandon Blood have a company called eMedFx: www.emedfx.com they have build a search engine that will allow you to enter the pharmaceuticals and neutraceuticals that a patient is taking to discover contraindications. I am going to see if they will allow E-Z Notes to use this data. I will let you know, but feel free to check out their web site.

diagnostic codes

It would be helpful if there was a way that when you add a diagnostic code on the travel card or superbill by using the other button you could chose to permanently add that code and description to the current list of codes instead of having to close out and go to set-up. I often add a code in "other" but forget to go back or am too busy at the time to go into set-up.

Here are two things I bet you did not know.

Changing the Onset

I want to address the patient who comes in stating that a month ago his neck was bothering him, you fixed it, but he screwed it up again. If his previous visit is in the E-Z Notes system, this is what you do. From the Start Screen, click on Regular visit, find the patient in the search box, and select him. You can hit the button, Was better, now worse, and select the body part, in this case, neck. Do your normal note, and when you get to the SuperBill, hit the button for New DX. Now select, Modifying Existing DX. You do not need to change the codes, only hit Done. This will keep the diagnosis codes the same, but will allow you to change the onset. Pick a new onset using the calendar as you always do. You will notice at the end of the note it will now say something like, The patient's diagnosis from Wednesday, May 30, 2007 was amended today and you will see the same diagnosis codes with a different onset. Its that easy, and it make your not more accurate.

Tender Taut Fibers

As you probably noticed on 8-15-08, Russ released version 08.17 and made it possible to indicate the quality i.e. mild, moderate, severe of the taut tender fibers (TTFs) for example if the patient has multiple problems: neck, right scapula and lumbosacral junction, you can now indicate that the neck has severe TTFs, the right scapula has mild TTFs and the L/S junction has moderate TTFs. We had received various requests for this, and it is now possible to do by clicking the either: No Quality, Mild, Moderate, or Severe, and then click the body part, i.e. select severe and then select neck. Play with a little it will get easy. You see, this is another case of, you asked for it you got it!

If the patient has an 800.xx diagnosis code entered, it would be nice to have eznotes prompt for accident information automatically.
In X-Ray reporting, there is a the that does not make sense grammatically automatically entered, and I would like to see it removed. For example: This is an imaging report taken on Saturday, August 09, 2008 on the patient, patient name, at the practice name with the following views: In our office, it says "...at the Lukert Chiropractic and Wellness with the following views:"
When clicking on the davis series in x-ray reporting, the neutral of neutral lateral cervical is spelled wrong in the notes.
